(RN) Admissions Nurse Responsibilities (RN) Admissions Nurse:
Assessment:
Conducting initial assessments of potential residents' medical history, physical condition, and care needs.
This assessment helps determine whether the nursing home can meet the resident's needs effectively.
Documentation:
Gathering and reviewing medical records, prescriptions, and any other relevant documentation to ensure accurate and up-to-date information is available for the resident's care plan.
Communication:
Interacting with prospective residents and their families to provide information about the facility's services, policies, and procedures.
Addressing any questions or concerns they may have and helping them understand the admission process.
Care Planning:
Collaborating with the interdisciplinary team, which may include physicians, nurses, social workers, and therapists, to develop an individualized care plan tailored to the resident's specific needs and preferences.
Coordination:
Coordinating with other healthcare providers to facilitate a seamless transfer of care, especially if the resident is transitioning from a hospital or another healthcare setting.
Medication Review:
Reviewing the resident's medication regimen to ensure accuracy and to make any necessary adjustments based on the nursing home's policies and the resident's care plan.
Legal and Regulatory Compliance:
Ensuring that all required legal and regulatory documents are completed accurately and in a timely manner.
This includes obtaining consent forms, insurance information, and any other necessary paperwork.
Facility Orientation:
Providing new residents and their families with an orientation to the nursing home's facilities, staff, and services to help them become familiar with their new living environment.
Emotional Support:
Recognizing the potential challenges and emotions that both residents and their families may experience during the transition process and offering emotional support and reassurance.
Follow-Up:
Monitoring the resident's well-being and adjustment during the initial days and weeks after admission, and making any necessary adjustments to the care plan based on their evolving needs.
Qualifications (RN) Admissions Nurse:
RN License in PA Positive and energetic individual with creativity Ability to work independently or part of a group.
